Job Description:
Join a nurse-led team dedicated to providing compassionate
hospice care in a supportive, family-like environment.
Administer skilled nursing and case management to terminally ill
patients at home or in facilities.
Assess, plan, and evaluate patient care, collaborating with an
interdisciplinary team.
Provide emotional support during end-of-life and bereavement
periods.
Qualifications include RN license, one year clinical experience,
and strong assessment skills; BSN preferred.

Work hours are weekdays with weekend and on-call rotations;
location is Grand Rapids area.
