Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 3 years of inpatient hospital/acute care experience required; EPIC charting preferred. License: RN state license required Certifications: BLS preferred Must-Have: - RN or Social Worker experience in inpatient case management - EPIC charting experience Description: The Case Manager RN will work a day shift with no weekends. This position is focused on inpatient case management, providing coordination of medical home care and addressing social issues for safe discharges. The average daily census is approximately 15 cases. The role involves conducting needs assessments and managing discharge planning. Familiarity with disease management and care coordination is essential for this position. Traveling between facilities may be required within a 10-mile radius. While weekends are not required, there may be opportunities to work additional shifts if desired. Candidates must be comfortable floating between facilities as necessary and without restrictions. Healthcare experience is essential, with an emphasis on meeting patient care standards and regulatory compliance.
